Abstract
Information systems are very useful for the world of education for management of student data and management of teaching and learning processes through monitoring student activities. Where storing data on students, teachers and academic data at SMK Negeri 3 Sekayu is still inputted to the computer using Microsoft Excel. Information on assignment values, daily test scores, mid semester and semester exam scores can only be known to students and parents only when distributing report cards. It is also the case with student attendance information that parents cannot know. In this case, with the Academic Monitoring and Student Achievement Information System to facilitate the school in inputting data, attendance every day, helping students to have an overview of the results of the learning process, in addition the system was built to help parents monitor students to see the development of students during learning and can find out the achievements that have been obtained by students. For this reason, in conducting these activities, SMK Negeri 3 Sekayu must have a system that can help and facilitate the activities of student learning. In this system, it is built using the Waterfall method and the PHP programming language and MYSQL database. While for system design used using structured programming modeling techniques in which there are Data Flow Diagrams (DFD) and Entity Relational Diagrams (ERD), and testing the system using Blackbox testing.
